IVUS & OCT — Intravascular Imaging

Seeing the artery wall from inside to size the stent exactly and expand it fully.

What is it?

IVUS is a miniature ultrasound probe and OCT a very high-resolution optical technology; both give cross-sectional images from inside the artery, showing true vessel size, lesion length, calcium and stent expansion.

Who needs it?

Left main disease, long and bifurcation lesions, heavy calcium, in-stent restenosis and any case where angiography alone is ambiguous.

How is it done?

The imaging catheter is advanced over the wire beyond the lesion and pulled back automatically within seconds. The images guide balloon and stent sizing and final optimisation.

Before the procedure

No extra preparation beyond that for angioplasty.

After & recovery

Large international trials show imaging-guided PCI reduces cardiac events and repeat procedures.

Risks & safety

Very safe; OCT needs a small extra amount of contrast, used cautiously in kidney disease.

Frequently asked questions

IVUS or OCT?

Both are valuable: OCT has higher resolution; IVUS penetrates deeper and needs less contrast.

Does it prolong the procedure?

Usually by only a few minutes.
